A 50yr female presents to the ER with c/o of diffuse abdominal pain, nausea, gradually decreasing urine output over t past 1 yr. O/E: GC: stable. Fair. BP: 150/100mmHg. Pulse: 82/min. P/A: Soft. No organomegaly. No tenderness. Chest clear. CBC: Hb- 9.2gm%, WBC- 12000, KFT: BUN- 90, Creat- 2.6, LFT:WNL, Urine R/M: Albumin ++, Pus Cells- 10-15, RBC- 8-10. X- Ray Abdomen: multiple b/l circular shadows at the level of L3- L5. USG Abd: s/o Adult PCKD. CECT Abd: b/l polycystic kidneys with multiple large cysts in both kidneys, largest measuring 4cm. Cortex thinned out on both kidneys.
